2023 Fire Island Jr. Lifeguard Registration Form

A PDF of the Registration Form can be accessed below. Fill out form entirely, please add your child's t-shirt size on the form. Each child will get a complimentary t-shirt as part of the program. Sizes include: Youth Med, Youth Large, Adult S, M, L, XL. Act now as registration typically reaches max participants and closes out by March!!
